+ : douleur ou craquements audibles ou perceptibles. traduction : specific tests for shoulder examinations • presented by sébastien ruiz • november 17th 2006 matthieu cesar, a hospital intern from the department of orthopaedic surgery, upper limb and hand surgery, under the direction of professor chammas, gives us here a small reminder of the tests that might help us when making a clinical evaluation of the shoulder. conflict tests ▪ the neer test : patient upright, scapula held in one hand and forward passive elevation carried out by the examiner. test positive when : forward or lateral pain typically between 90° and 140°. ▪ the hawkins test : patient upright, arms at 90° forward elevation, the examiner evaluates the internal rotation, elbow at 90°. test positive when : pain during this manœuvre (=conflict between the trochiter and the acromioclavicular ligament). ▪ the yocum test : the patient places his hand on the opposite shoulder. he then attempts to raise his elbow while the examiner places a downward force against it. test positive when : pain during the manœuvre. ▪ the cross-body adduction test : patient upright, arms at 90° forward elevation, passive adduction by the examiner. test positive when : pain on adduction (= often acromioclavicular damage). rotator cuff tests ▪ the jobe test (or « empty can » test): arms at 90° abduction in the plane of the scapula with the thumbs pointing downwards (= internal rotation). the patient must resist a downward force exerted by the examiner. test positive when : no resistance, the upper limb comes back to the side of the body. ▪ the patte test: the patient is upright with 90° forward elevation in the plane of the scapula. test positive when : external rotation is difficult and impossible to resist the imposed internal rotation. ▪ the lift-off test : the patient is upright with one hand behind the back. should be able to raise the hand off the flat of his back or resister pressure from the examiner. test positive when : inability to carry out the two previous 2 manœuvres. ▪ the belly-press test : patient has his hand on the belly. should be able to move his elbow forwards without removing his hand from his abdomen. test positive when : inability to carry out this manœuvre. ▪ the « hornblower » sign : the absence of external rotation means that the arm is abducted when trying to raise the hand to the mouth. ▪ « dropping sign » =doorway sign : arm alongside the body, elbow at 90° flexion. the examiner progressively brings the forearm to 45° external rotation, where he then lets go. the patient should be able to maintain this position. test positive when : return of the forearm to 0° external rotation. tests for damage to the long head of the biceps or the labrum ▪ the speed test = palm-up test : arm at 90° forward elevation, elbow extended with the palm upwards . difficulty in raising the arm forwards. test positive when : pain in the long head of the biceps. ▪ the yergason test :arm alongside the body, elbow at 90° flexion. patient tries to supinate the arm while the examiner resists at the same time as he palpates the bicipital groove. test positive when : pain in the long head of the biceps. ▪ the anterior apprehension test: patient lying down, the arm is abducted to 90° and rotated externally. test positive when : pain during this manœuvre. ▪ the o’brien test: the patient’s arm is fully extended, then adducted to 10° in the sagittal plane. the arm is maximally internally rotated, and the patient tries to resist against the examiner’s downward force. 1st test positive when : pain deep down in the shoulder. 2nd to see whether the pain decreases in the 2nd ▪ the jobe relocation test : patient lying down, abduction + external rotation of the arm and the examiner evaluates the backward sliding movement, then releases the pressure test positive when : pain on releasing. ▪ the crank test : patient lying down, complete abduction. the examiner tests external rotation and internal rotation in succession. test positive when : pain or a « clunk » feeling or sound . part : thumb pointing downwards. part of the test. part :thumb pointing upwards. this is articles membre inférieur
